I understand that my participation in this activity being offered by Middle Way LA (doing business as Inner Way LA) is completely voluntary and at my own risk. I agree to hold Middle Way LA and all of its officers, agents, representatives, members, volunteers, successors and assigns completely harmless of and from any and all liability if I sustain any damage or injuries in connection with or related in any way to my participation in this activity. By signing this waiver, I attest that I am older than eighteen years of age.  I agree that I, on behalf of myself and my successors, assigns, heirs, distributees, guardians, and legal representatives, will not make any claim against or sue Middle Way LA for damages, including but not limited to any and all claims for bodily injury, emotional distress, or wrongful death, regardless of whether or not negligence can be demonstrated, and I release Middle Way LA from any and all damages suffered by me. The only exclusion from this release is any claim solely and directly resulting from the gross negligence or intentional acts of Middle Way LA. 

Any dispute, claim or controversy arising out of or related to this agreement, including the determination of the scope or applicability of this agreement to arbitrate, shall be determined by arbitration in Los Angeles, California before a single neutral arbitrator. The parties shall select the arbitrator pursuant to Judicate West's rules. The arbitration shall be administered pursuant to Code of Civil Procedure section 1280, et seq. The decision of the arbitrator shall be neutral and binding.
